NEW PATIENT FORMS
Please print and fill out the necessary forms and bring them to your first appointment. Call the office at 248-354-0967 if you are unsure which forms to bring. They will give you all necessary information so you feel prepared for your appointment! If you are unable to bring the forms, please come 15 minutes early to allow enough time to complete the forms prior to your appointment.
We still ask that even if you do have your forms, please arrive 10 minutes earlier than your scheduled appointment. We may have other paperwork for you to fill out and our staff can prepare your medical chart. It is *very important* that you bring all insurance cards and photo identification to your child's appointment.
